Gilead kondigt fase 1-gegevens aan van onderzoekstherapie (GS-6615) bij patiënten met lange QT-3-syndroom

Gilead Sciences, Inc. (Nasdaq: GILD) kondigde vandaag de resultaten aan van een fase 1 klinisch onderzoek (GS-US-279-0110) van GS-6615, een experimentele, selectieve remmer van de late natriumstroom, waarbij een verkorting van het QTc-interval wordt aangetoond (de tijd tussen het begin van de Q-golf en het eind van de T-golf in de elektrische cyclus van het hart) bij patiënten met lange QT-3-syndroom (LQT3). LQT3 is een genetische aandoening die het QTc-interval van het hart verlengt en levensbedreigende hartritmestoornissen (abnormale hartslag) kan veroorzaken. De resultaten van dit onderzoek (uittreksel #AB34-05) worden vandaag in San Francisco tijdens de jaarlijkse bijeenkomst van de Heart Rhythm Society gepresenteerd.

De aangeboren lange QT-tijd-syndromen (LQTS) bestaan uit een groep erfelijke aandoeningen die het elektrische systeem van het hart aantasten, en worden gekenmerkt door een onregelmatige of snelle hartslag die kan leiden tot syncope (flauwvallen), hartstilstand of plotselinge hartdood. Normale QTc-intervallen variëren van 380-460 milliseconden (ms). Patiënten met LQTS hebben een QTc die meestal boven 470 ms ligt. LQTS-symptomen kunnen zich voordoen bij zware lichamelijke inspanning, emotionele stimulatie of tijdens het slapen. De meest voorkomende typen zijn LQT1, LQT2 en LQT3. LQT3 wordt in verband gebracht met een mutatie in het gen dat het natriumkanaal van het hart (SCN5A) codeert. Er bestaan op het moment geen goedgekeurde geneesmiddelen voor patiënten met LQT3.

"In de afgelopen decennia is de kennis over LQTS sterk toegenomen, waaronder ons begrip van de genetica die geassocieerd wordt met verschillende vormen van de ziekte", aldus Arthur J. Moss, MD, hoogleraar cardiologie, Medische Faculteit, University of Rochester Medical Center, Rochester, New York. "Een belangrijk voorbeeld van deze vooruitgang is de ontdekking dat LQT3 wordt veroorzaakt door SC5NA-mutaties in de natriumkanalen van het hart. Ontdekkingen zoals deze hebben geholpen bij de ontdekking en ontwikkeling van hoognodige nieuwe behandelingen voor patiënten met deze levensbedreigende ziekte."

In dit onderzoek werden tien patiënten met LQT3 geëvalueerd in het Clinical Research Center van Universiteit van Rochester, waarbij ze enkelvoudige orale doses van GS-6615 toegediend kregen, variërend van 10 mg tot 60 mg. Bij het onderzoek waren patiënten betrokken die een QTc van meer dan 480 ms hadden. De QTc voorafgaande aan de dosering (dag -1) werd vergeleken met de QTc tijdens het geneesmiddel (dag 1) gedurende overeenkomende perioden van 12 uur. Tijdens het onderzoek werd bij alle doseringen een verkorting van de QTc waargenomen, met een maximale verkorting van de QTc variërend van -44 ms tot -80 ms. Er werden bij het toedienen van GS-6615 geen veiligheidsproblemen waargenomen.

"De verkorting van de QTc in dit onderzoek biedt klinisch bewijs dat suggereert dat GS-6615 een remmer is van de late natriumstroom en een belangrijke rol kan spelen bij de behandeling van patiënten met LQT3", aldus Wojciech Zareba, MD, PhD, hoofdonderzoeker voor het fase 1-onderzoek naar GS-6615 en hoogleraar in de geneeskunde, Director van Cardiology Clinical Research en Director van het Heart Research Follow-Up Program aan de University of Rochester Medical Center.

Op grond van deze resultaten is Gilead van plan om later dit jaar een fase 2-onderzoek naar GS-6615 bij patiënten met LQT3 te beginnen. Op grond van preklinische gegevens voor GS-6615 en klinische gegevens met betrekking tot de rol van remming van de late natriumstroom bij andere cardiovasculaire ziekten, is Gilead bovendien van plan om fase 2 klinische onderzoeken te beginnen bij patiënten met hypertrofische cardiomyopathie (HCM) en ventriculaire tachycardie en ventrikelfibrilleren (VT/VF).

GS-6615 is een krachtige en selectieve remmer van de late natriumstroom van het hart, die geassocieerd wordt met verschillende genetische en verworven cardiovasculaire aandoeningen, waaronder lange QT-3-syndroom (LQT3), hypertrofische cardiomyopathie (HCM) en ventriculaire tachycardie en ventrikelfibrilleren (VT/VF).
