Live-OP auf der TCT führt individuelle Simulationsprobe mit PRS vor

Das Simbionix PROcedure™ Rehearsal Studio (PRS) wurde auf die Probe gestellt, als Barry T. Katzen, MD, FACR, FACC, FSIR, Gründer/Medizinischer Leiter des Baptist Cardiac and Vascular Institute die Technologie für einen präoperativen Plan und eine Probe für eine Live-OP nutzte, die er anlässlich der 2012 TCT Conference durchführen würde. Nach der Probe, die im Vorfeld der Operation durchgeführt wurde, führte Dr. Katzen die endovaskuläre Aneurysmareparatur (EVAR) live auf der TCT durch. Diese Veranstaltung mit dem Titel "Future Techniques for Endovascular Therapy" (Zukünftige Techniken für Endovaskulartherapie) diente der Vorführung neuer Techniken für mögliche Verbesserungen zu einer Reihe an Produkten im Bereich der Endovaskulartherapie.

Die Ausbildungssitzung umfasste die präoperative Beurteilung der Anatomie des Patienten, die Auswahl von Werkzeugen für das EVAR-Verfahren und auch den Einsatz einer Gore C3 Excluder-Endoprothese. Der Patient wies eine anspruchsvolle Anatomie mit viel Iliacaltortuosität und einem ziemlich großen Durchflußlumen auf, was für die Ärzte eine technische Herausforderung darstellte.  

Die Computertomografie (CT) des Patienten wurde in das PRS heruntergeladen. So konnte Dr. Katzen ein patientenspezifisches, virtuelles 3D-Anatomiemodell erstellen. Dr. Katzen verwendete dieses, um den endovaskulären Eingriff zu planen. Dazu gehören die geeignete Strategie für den Zugang, die Auswahl von Werkzeugen, Führungsdrähten, Kathetern, der Endoprothese sowie die Wahl der effektiven Durchleuchtung.

Anlässlich der TCT führte Dr. Katzen das Ausbildungsvideo vor und führte im Anschluss daran die eigentliche Operation an dem Patienten durch, die zuvor geprobt worden war. Die Live-OP replizierte den Probedurchlauf. http://simbionix.com/simbionix_videos/tct-2012-evar-live-case-video/

"Als Ergebnis der PRS-Simulation bewerteten wir unsere Messungen und modifizierten die rechte Gliedmaßen-Seite. Wir überprüften alles, begannen von vorne und berechneten alles neu und stellten so im Grunde die richtige Gliedmaßen-Größe ein. Wir waren davon überzeugt, dass es funktionieren würde, und das hat es auch", sagte Dr. Katzen. "Die Simulation hat ein Endoleak vom Typ Ib in der rechten Extremität prognostiziert, die wir mit einem Ballon korrigierten und so kein weiteres Transplantat mehr benötigten."
